1. ACKNOWLEDGMENT OF ACTIVITY
I understand that the GET HOT LLC Youth Performance League ("YPL") is a physical fitness, athletic development, and performance training program involving, but not limited to:
Running
Sprinting
Agility drills
Shuttle runs
Standing long jumps
Calisthenics
Bodyweight exercises
Team activities
Strength and conditioning exercises
Outdoor fitness activities
Physical assessments and testing
I understand these activities involve inherent risks.
2. ASSUMPTION OF RISK
I knowingly and voluntarily assume all risks associated with participation, including but not limited to:
Falls
Slips
Trips
Muscle strains
Sprains
Fractures
Heat exhaustion
Heat stroke
Dehydration
Concussions
Cardiac events
Insect bites
Allergic reactions
Equipment failure
Acts of other participants
Natural hazards
Permanent disability
Death
I understand that these risks may arise from known or unknown causes.
3. RELEASE OF LIABILITY
To the fullest extent permitted by Florida law, I release and forever discharge:
GET HOT LLC
and its:
Owners
Officers
Directors
Employees
Coaches
Trainers
Volunteers
Independent Contractors
Sponsors
Landowners
Facility Operators
Event Partners
from any and all claims, demands, damages, causes of action, liabilities, costs, attorney's fees, and expenses arising from participation in the Youth Performance League.
This release applies whether such claims are known or unknown, foreseen or unforeseen.
4. NEGLIGENCE WAIVER
I specifically understand and agree that this release includes claims alleging ordinary negligence by GET HOT LLC and its representatives.
I understand that I am giving up substantial legal rights by signing this agreement.
Nothing in this agreement shall release claims involving:
Gross negligence
Reckless misconduct
Intentional misconduct
Conduct prohibited by Florida law

7. MEDICAL AUTHORIZATION
In the event of illness or injury, I authorize GET HOT LLC personnel to obtain emergency medical treatment for the participant.
I understand that GET HOT LLC is not responsible for medical expenses incurred.
I agree to pay all medical costs resulting from participation.
8. CONCUSSION AND HEAD INJURY ACKNOWLEDGMENT
I understand that participation in athletic activities may result in concussion or traumatic brain injury.
I agree to immediately report symptoms including:
Headache
Dizziness
Nausea
Confusion
Vision problems
Loss of consciousness
I understand that participants suspected of a concussion may be removed from activity until medically cleared.
9. COVID-19 AND COMMUNICABLE ILLNESS ACKNOWLEDGMENT
I understand that participation may expose the participant to communicable illnesses, including but not limited to viral or bacterial infections.
I voluntarily assume these risks.
10. PHOTO, VIDEO, AND MEDIA RELEASE
I grant GET HOT LLC permission to:

Record video
Record audio
Use images for advertising
Use images for websites
Use images for social media
Use images for promotional materials
without compensation.

11. CODE OF CONDUCT
The participant agrees to:
Follow coach instructions
Respect staff and participants
Use equipment properly
Refrain from bullying or harassment
Maintain appropriate sportsmanship
Failure to comply may result in removal without refund.
12. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless GET HOT LLC and its representatives from any claims, demands, lawsuits, losses, damages, attorney's fees, or expenses arising from:
Participant conduct
Participant negligence
Violations of rules
Claims brought by or on behalf of the participant
13. INSURANCE ACKNOWLEDGMENT
I understand that GET HOT LLC may not provide health, medical, accident, disability, or life insurance coverage for participants.
I understand it is my responsibility to maintain adequate insurance coverage.
14. SEVERABILITY
If any provision of this agreement is found unenforceable, the remaining provisions shall remain in full force and effect.
15. GOVERNING LAW
This agreement shall be governed by the laws of the State of Florida.
Venue shall be exclusively located in Hernando County, Florida.
16. ACKNOWLEDGMENT OF UNDERSTANDING
I have carefully read this entire agreement.
